APA Releases Proposed Revisions to Diagnostic Criteria for “Transgender Children”

The following are the proposed revisions to the diagnostic criteria for children exhibiting sex-role incongruence. If approved, they will be used to diagnose sex-role noncompliant children to be treated with medical “puberty suppression”, sterilization and extensive plastic surgery to change the child’s body to a newly dysfunctional but superficially rough visual approximation of the other sex. The APA’s position is that social sex-roles are biologically created, possibly by brain neurology which although incredibly plastic in every other instance, for some reason in terms of sex roles is unchangeable. They advocate sterilization and lifetime cross-sex hormone treatments for children that are unable or unwilling to adhere to sex-based gender stereotypes and traditions.

Gender Dysphoria (in Children)** [1]

A. A marked incongruence between one’s experienced/expressed gender and assigned gender, of at least 6 months duration, as manifested by at least 6* of the following indicators (including A1): [2, 3, 4]

1. a strong desire to be of the other gender or an insistence that he or she is the other gender (or some alternative gender different from one’s assigned gender) [5]

2. in boys, a strong preference for cross-dressing or simulating female attire; in girls, a strong preference for wearing only typical masculine clothing and a strong resistance to the wearing of typical feminine clothing [6]

3. a strong preference for cross-gender roles in make-believe or fantasy play [7]

4. a strong preference for the toys, games, or activities typical of the other gender [8]

5. a strong preference for playmates of the other gender [9]

6. in boys, a strong rejection of typically masculine toys, games, and activities and a strong avoidance of rough-and-tumble play; in girls, a strong rejection of typically feminine toys, games, and activities [10]

7. a strong dislike of one’s sexual anatomy [11]

8. a strong desire for the primary and/or secondary sex characteristics that match one’s experienced gender [12]

B. The condition is associated with clinically significant distress or impairment in social, occupational, or other important areas of functioning, or with a significantly increased risk of suffering, such as distress or disability.**

The proposed revisions rename the childhood diagnosis from the DSM4’s previous “Gender Identity Disorder” (GID) to the revised name “Gender Dysphoria”(GD).

Gender = Sex Role. Dysphoria = Distress.

The previous APA  revisions proposed to re-name the diagnosis “Gender Incongruence” (GI) :

 “In response to criticisms that the term was stigmatizing, we originally proposed to replace the term “Gender Identity Disorder” with Gender Incongruence. This was accompanied by a re-definition of the condition, revised criteria, eliminating the previous subtype pertaining to sexual attraction, and introducing a new subtype categorization that does not exclude individuals with a somatic disorder of sex development (DSD). We chose the new term, Gender Incongruence, as descriptive and to avoid a presupposition of the presence of a clinically significant acute distress in all cases as a requirement for the diagnosis. In part, this was based on more general discussions in the DSM-5 Task Force on separating out the distress/impairment criterion and evaluating these parameters as a separate dimensions.

 We also debated and discussed the merit of placing this condition in a special category apart from (formerly Axis-I) psychiatric diagnoses to reflect its unusual status as a mental condition treated with cross-sex hormones, gender reassignment surgery, and social and legal transition to the desired gender (particularly with regard to adolescents and adults). We chose not to make any decision between its categorization as a psychiatric or a medical condition and wished to avoid jeopardizing either insurance coverage or treatment access”

Also reinstated at the behest of transgenderists is the severity scale, which trans-activists feel assists with the authorization of sterilization and medicalization of children who experience distress conforming to sex-roles.

“This revised proposal also re-introduces a clinical significance criterion, B, which clarifies that diagnosis requires distress or impairment that meets a clinical threshold. This criterion is present in the DSM-IV but was removed from the first DSM5 proposal. Parents of affirmed/transitioned youth and care providers have raised concerns that removal of the clinical significance criterion would further obscure the medical necessity of puberty delaying medications as well as hormonal and surgical transition care.

Here is the APA’s clinical significance survey. Each question must be answered as shown:

Should religious or other parents be permitted to subject their children to “treatments” which prevent them from going through puberty and subsequent surgical sterilization because they exhibit distress about complying with sex-roles? Because they do not want to treated in the way boys and girls are treated – very differently according to sex- and reject  the roles expected of them, and enforced on them, even by violence, or medical violence in the case of the sterilization advocated by transgenderists and the APA? Should children distressed by sex-roles be diagnosed with a mental illness or “medical” condition even though they are perfectly healthy in every way prior to “treatment”, but not after, because the “treatment” is permanently disabling? Is it child abuse? Is it a human rights crime? We KNOW WITHOUT A DOUBT that the vast majority of these kids will acquire the ability to cope with their sex-role distress after going though natural puberty, without further need for psychiatric support, whether by finding social support among other sex-role rejecting people, or by fighting the nature of sex-roles and rejecting the roles entirely. We KNOW that MOST of these kids, left alone, grow up to be GAY, and well-adjusted in their communities. Should psychiatrists be “correcting” gender-nonconforming children? Should they be slating these kids for irreversible  sterilization and profound surgical genital mutilation? Should boys that want to have long hair and play with girls and hate sports and like dolls be pathologized? Should girls that don’t want to be treated as girls be “treated” with lifetime cross-hormones so they can look like boys? Or should the APA develop “treatments” that are non-invasive and that support children who reject the gender roles imposed on them? Should the APA fight sex-role conformity rather than promote it by pathologizing children?

The American Psychiatric Association requests public feedback on these proposed revisions. Deadline is June 12, 2011.

And the plastic surgeon who uses two colours for outlining barbie breasts, can the dude not tell left from right? Perhaps he should label them 1 and 2 so he does not accidentally do the same one twice or something. He certainly should not try things like walking and chewing gun simultaneously.

    What about children who won’t be trans or gay? Some years ago, there was a popular tv program, book and music called Free to be You and Me. Children played with all kinds of toys, were encouraged to see themselves as human, consider they could be more than just some role hammered at them by the patriarchy. Where are THOSE parents now? Especially, where are THOSE feminists who were behind gender non-conformity?

    This psych profession just gets more and more embarrassing to scientists every day.

    Wow, it’s child abuse for sure. What is the matter with people? Seems they’ll do anything just to continue oppressive sex roles in perpetuity.
    I was really angry when I read an article in my local paper a couple of years ago about a 13 year old girl in a local middle school who was being allowed to return to the school as a boy, complete with a name change. They even explained the whole thing to her class in a way that indoctrinated the kids into the trans ideology. Appalling! IMO, the adults who go along with that are either criminal or they have completely lost their ability to think critically.

    That’s sad and scary. And just plain wrong. As a kid I remember doing all those things, all those alleged symptoms of gender dysphoria. Believe it or not, there are thousands of little girls who don’t want to wear dresses or play with dolls, who want to be male in fantasy play (because they get to have the sword.) As a girl, I don’t know if it’s even possible to grow up without experiencing “dislike of your sexual anatomy.” Besides the media that constantly tells you there is something wrong with girly parts, there can be pain and discomfort involved.

    The mental illness, the dysfunction here, is that we have parents, the APA, that seems to believe that gender is identity, that gender is all that kids should be focused on, that it is what defines them. Whatever happened to all those other thousands of skills and talents that we’re supposed to be nurturing in children? Gender is not identity, it does not define who you are. Kids will go through phases where they believe a pink sweater or big ears is what defines them and it’s the job of adults to point out all the other millions of things that make up a human being. I can’t imagine somebody saying, yep, your big ears are all that define you as a person, so we’d better chop them off and teach you how to act like a small eared person.

      I agree. And yet now, when a child is not gender-conforming, she or her parents get the message that maybe she is trans, rather than normal or intelligent to be rejecting oppressive gender roles. So then the child or parent become convinced and entrenched in that belief, because the view that the child must be trans is the only message out there these days, regarding non-conforming people, rather than the feminist view that roles are patriarchal social constructs. What the kids and parents are exposed to become their belief system.

      It reminds me of the days when lesbians believed from Freud that they either had to be butch or femme, and became entrenched in those identities and believed they were “true.” (which, of course, has returned since the 1980s.)

    This GID treatment schema appears to be mixed in with some kind of unholy alliance in the ongoing psychiatric race to pathologize deviant or unwanted behaviour expression from a child.

    My thoughts on this are that *it* is *all* being done (of course!) in the name of getting them *treated* (the earlier the better), lest they *suffer* from their obvious! GID, even more!

    There is nothing really *radical* or *transgressive* about blind submission and psycho-social slavery to the gender binary; as evidenced by this gendered behaviour policing in children these days (got to get them hooked into the medical system while they are young! It makes for good, loyal, and more importantly lifer customers).

    This situation, by all accounts, (including those from your own blogging, GM), is only increasing severity and significance. Consequently, this GID pony-ride and hoop-show is getting more and more(!) out of hand, (damaging more and more children for life) the longer that it persists. That is, this a psychosocial and cultural *movement* which is *moving* further away from *sanity* and *reason* and not towards.

      Yes, on one level, you would think so, because it is sending non-conforming males under the knife. But at the same time, trans is consistent with the gay politics of embracing patriarchal institutions, even worshipping them. Because the major gay political organizations have, for at least four decades, focused on being accepted *into* patriarchal institutions (marriage, parenthood, military, corporate america, etc.) rather than rejecting them as inherently sexist and oppressive. That’s because gays experience homophobia from straight men, but they don’t experience sexism (sexist oppression.)
      So the political organizations have not been interested in challenging patriarchy, just patriarchy’s homophobia: that one little piece of patriarchy. Other than that, gay organizations have pretty much been a-ok with continuing the patriarchy.

      This is discussed in Marilyn Frye’s book, The Politics of Reality, her essay on “Lesbian Feminism and the Gay Rights Movement: Another View of Male Supremacy, Another Separatism,” which is really worth getting her book for and reading that essay. Actually, all of the essays in that book.

      Also, gays are statistically much more likely to be aware of homosexual feelings–and exclusively such feelings–from a very young age, thanlesbians. So gays are more likely to have a perception and politics of being “born this way,” which is the belief in trans politics. For more on this, a fantastic book is “Queer By Choice: Lesbians, Gay Men, and the Politics of Identity,” by Vera Whisman. Brilliant feminist book.

    TLC just did an episode called “Right Guy, Wrong Body” that was an atrocious example of homophobia. This woman, Nat, was convinced she was not a lesbian because she was only attracted to straight women, “in the right way.” Every other sentence was a declaration of her heterosexuality and a desire to be “normal.” She was in a relationship with a conservatively raised woman who didn’t want to be with Nat until she was fully transitioned so they could be a “normal heterosexual” couple. Naturally the pyschiatrist, her parents, the medical profession, all said the only cure for Nat’s obvious “gender dysphoria” was immediate radical surgery.

    It was really a heartbreaking show to watch. This couple was so young and so confused and so convinced that this was what they had to do to be together. TLC didn’t portray it as painful, as a tragedy to watch, they presented it as conquoring a birth defect so this couple could present to the world as “normal” (read, not gay.)

    I’ve known that the trans-movement was about erasing lesbians, but I’ve never seen it so personal, so blatant, so unapologetic, as if this were just the right thing to do.

      Im sorry my episode came off that way, and you are right. Some things came out wrong and I never meant it to sound like I was trying to be “Normal” cuz there is not such thing. Basically I am just trying to be the complete man I am inside and have the outside match. That’s all. I was in a terrible relationship with someone who couldn’t except me till all my parts matched. And thats her own problem. The only way that I would feel happy and content in my own skin was to change my body to match my mind. I understand that some ppl are ok with their bodies. That’s how they feel so they are lucky cuz the dont have to have a sex change. My family just wants me to be happy. I have a sister who is lesbian. Shes amazing the way she is cuz shes comfortable. And now I am too after having my sex change. I 200% support all GLBTQ people in all their forms. And this is mine.

    Just so everyone knows the context, pathologizing incorrect gender behavior has been in the DSM in editions before now. You can find info online about the language in IV, which came out in 1994. And in all that time, the APA has not only not listened to those who criticize this pathologizing, it has ramped it up.

    In IV, they also linked gender trouble with homosexuality. They haven’t bothered to hide their conservatism, it’s been there all along.

But in order to get the body mods they want they need to not only essentialize sex roles but medicalize them. They want gender non-compliance to be categorized as a physical birth defect of the brain, and also to assert the surprisingly unscientific notion that brains and neurology are unchanging, so the body must be changed to comply with the unchangeable brain (which likes Barbies and lingerie, yo). Their problem is that gender doesn’t exist. Is not quantifiable or even definable. There is no scientific evidence. There is no “gender deformity” requiring medical intervention. So they are stuck needing psychiatrists for the body mods they want because mental illness is the only provable cause for gender essentialism that requires lifelong drug dependence and genital mutilation as a “cure”.

        If you’re not a cookie-cutter conventionally feminine girl or masculine boy, you have to choose your parents especially carefully these days. It used to be enough just to slip the stork a few bucks so it wouldn’t leave you on some ultraconservative couple’s doorstep.

        Now you have to take steps to ensure you don’t end up with hip, progressive, queer or queer-ally parents, either. They’d NEVER ship you off to Dr. Dobson’s degaying camp, but out of loving concern they just might put you on a fast track to gonad-shriveling drugs, synthetic hormones, and the removal of healthy body parts.

        It would also be smart for anyone planning to be born to wait a couple decades until this gender insanity passes. Of course, you’ll have to deal with global warming, but life is a series of trade-offs.

        I just hope Shiloh did her due diligence prior to conception. Now that she’s here, she’s stuck with everything that goes along with being a (perfectly normal) non-pinkbarbieprincessified four-year-old girl in the 2010’s. So far, her parents don’t seem to be susceptible to pressure where she’s concerned — and they’ve been under a lot. Let’s hope they queergressives don’t get to them where the old-fashioned sexists didn’t.

    You can not shame me with my chromosomal assignment. I know that I am neither female nor girl, and hence, not gender: woman. I will never deny that I was once saddled with male privilege (and junk) and a MAB life and I admit that I called myself a MAN, a male and a dude for two and half decades.

    It is true that I had a male name and an “M” on my dox for many years. I will never not be aware of my un-femaleness. I know my (gender political) place and I have no illusions about my artificial plastic-ness. Though (faux) “girl” may be accurate, because my faux-girl wannabe faux-FAAB lived experiences is (I am guessing) only a tiny fraction of your (or any) real-woman lived life, GM.

    With respect GM, may I submit that I do not fancy myself as female. I am lucky that the State recognizes me legally as such, but, I just want to be female, so I do consciously try to emulate and mimic expected social and gender behavior from real ones, to the heartfelt best of my ability. But I do not insist, nor demand, that you (or any wbw), accept that I am in fact a: Real Female™.

    Why wouldn’t you like to see little Shiloh transified?

    Speaking only for myself, GM, there is a very good reason that I would not like to see any child *transified*. And it is this. You yourself have stated that you can not put your finger on lived gender. You can not point it out in a genetic test or in a PET scan. But we all, all of us as human beings, have differing penchants, inner-orientations and hereditary dispositions.

    So I personally believe that children must be allowed to becomes teens, and then mature into adults without medically unnecessary chemical or psychological or surgical coercion or labeling or reassignment, just on the chance that the child may end up permanently wanting to slot themselves into a specific gender role they may appear to be childishly experimenting with and acting out, despite whatever their born sex or chromosomal assignment may indicate. Because, what if, these parents and gender binary cops (Pdocs and Pshrinks), are truly wrong, years or even decades down the line? Who will take responsibility for a child transitioners future adult suicide or detransition, when the child was not legally able to be truly informed and truly consenting to early transition and resocialistion in the first place!!!???11.

    Thus, I honestly think it is a crime of child abuse and mutilation, really, to label a little boy who seems to gravitate to typically girly things, or diagnose a little girl who seems to gravitate towards typically doodly activities or persuasions with an unknown neurological birth defect which can not be clinically proven in any way. This is a real social injustice. It is, (imo) unethical child experimentation is what it really is.

    It is very clear that the eventual outcome of this gender and social“experiment” will be tons more SRS!regretters. With any luck, as the phenomena and crime of child-rape and binary gender conservationism that is this Trend of summarily diagnosing and transitioning (treating) otherwise sane and healthy and naturally femme or butch gay and lesbian children into artificially reconstructed Plastic Girls and Plastic Boys will (maybe) finally clue these genetic determinist biopsychs (and the twans that worship mouths gaping at their male-altars) that they are doing sex and gender WRONG!!!11.     I know I am taking up all sorts of space with my unasked for dissertation here (not mabish at all!) and I beg your pardon, but I would like to wrap myself up by rendering my own personal take on this TransTrending and child sexual and social mutilation phenomenon.

    It is and should be an adult earned privilege to voluntarily submit to being Plasticized into fauxMAB or FauxFAB transhumanism. It should be done with great consideration and with plenty of time to learn to live with and accept (or ultimately reject) being one’s sex and birth-assigned gender role without being pressured or coerced to transform or transition by adults whom a child clearly looks up to, wants to please and gain validation and reassurance from (therein lies the real mind, body and soul life-rape of parent-approved, doctor-ordered childhood or teenaged medical transitioning, imo).

    I think that for a person who truly wants in on the Brotherhood and Sisterhood of Transhumanism, that each and every mtf and ftm be required or at least strongly encouraged to identify and resolve possible comorbid mental, emotional and spiritual health problems.

    It really would be in every wannabe transperons best mental and physical interests to be addressing their possible teenage-or-childhood-induced BDD and get any gender-role confusion or body-sex envy resolved or therapied so they don’t transition and ultimately live to regret it.

    In my humble opinion, there is nothing wrong with wanting to be chemically adjusted or hormonally or sexually reassigned for personal tastes as a mature adult exercising voluntary choice. Choice, and (willing) informed consent being the main issue for me, that puts me in a position of near, if not total agreement here with you, GallusMag. Thank you, again.

      We disagree on major points about whether adults should medicalize sex-roles. Especially those trans at the top of the sex hierarchy (men) who not only mutilate themselves but colonize the underclass socially and by pressing for legal protections which prioritize “gender identity”/”sex-role identity” (men) over actual physical sex. But I appreciate your willingness to share your thoughts. And agree with the ideas you articulated about the inability of a child to give informed consent and the grave human rights violations conducted via these controversial medical measures. I hope to hear from more trans who have thoughts which don’t cling to the typical memorized trans ‘splanation cliches.

      Yes, it is such a privilege for a four-year-old girl to have her appearance picked apart and scrutinized in this manner by every tabloid under the sun, and all over the sleepless glowing expanse of the interwebs. To have thousands of hostile strangers calling her freakish and fucked-up, and her parents extra-fucked-up for not forcing her into a pink dress. And to have thousands of sympathetic strangers speculating about her “gender identity,” and whether or not her non-pink-dress-wearing habits means she should someday undergo a series of bizarre medical procedures to turn her into a simulacrum of a man. Then there’s the super-duper privilege of having the obviously self-hating and misogynistic Chaz Bono turn her into a kind of side attraction in Chaz’s own F2T dog-and-pony show. Hoo boy, that’s privilege, I tell ya.

      Meanwhile, out there in non-celebrity I Hate Pink Dresses Land, there are thousands of girls dealing with much the same thing, just on a smaller scale and out of the public eye. Hostile, scrutiny, unwanted and inappropriate speculation, and in some cases, dangerously misguided ‘help’ from the gender-preoccupied. Also, some of them are getting beaten up regularly for their gender non-compliance. It’s a myth that such things only happen to sissy boys. Shiloh’s probably missing out on that fabulous privilege, because of who her parents are, but I don’t know if that really compensates for having Chaz raise the possibility of her future F2T transition in the national news.

      In additions to all the enviable forms of cis-privilege described above, those non-famous non-girly girls are also getting the standard girl-treatment, i.e., being trivialized, sexualized, and harassed, at the same time they’re also getting hell for not being conventionally feminine enough. Those two things can and do coexist.

      The comments over at Bilerico and Feministifying are atrocious. A lot of them just come right out and say Chaz’s misogyny is fine and dandy, because it’s part of Chaz’s journey into Chaz’s new identity as a person with sideburns. Nice to know what the funfems and queer really think about women, isn’t it?
